Forms for registration

During the registration process, be prepared to upload the following documents:


1)    Proof of residency within GNO JPRD boundaries (driver's license, homeowner or tenant
        records, utility bills)

OR

        A form verifying attendance at a school within GNO JPRD boundaries. The school enrollment
        form can be found here.


2) Birth certificate of participant


3) Medical release form. The medical release form can be downloaded here.
